What to Expect: Your 7-Phase Treatment Timeline
One of the most important things we can do for our patients is set accurate expectations. WJ-MSC therapy is not a quick fix — it is a biological process that unfolds over months. Understanding each phase of your healing journey helps you stay the course, recognize progress, and know when to reach out. This is your complete roadmap.
Phase 1: Evaluation & Planning
Weeks 1–2 Before Treatment
Your treatment begins before any cells are administered. The evaluation phase is where we determine whether WJ-MSC therapy is appropriate for your specific condition, severity, and goals — and where we build your individualized treatment plan.
- Comprehensive Consultation: Dr. Kelton reviews your complete medical history, current medications, prior treatments, and treatment goals. This is a two-way conversation — we want to understand what matters most to you.
- Imaging Review: We review your existing MRI, X-ray, or ultrasound imaging. If current imaging is not available or is outdated, we will request updated studies before proceeding. We do not treat without objective confirmation of your diagnosis.
- Realistic Outcomes Discussion: We review what the evidence shows for your specific condition and severity, what a realistic best-case outcome looks like, and what the trajectory for experiencing benefit typically is.
- Treatment Planning: If you proceed, we determine the appropriate cell dose, delivery method (intra-articular, intravenous, intrathecal, or topical), and post-treatment protocol specific to your condition.
Phase 2: Treatment Day
What Happens When You Arrive
Treatment day is straightforward. Most patients are in and out within 2–3 hours.
- Check-In & Pre-Treatment Briefing: You will review and sign informed consent documentation. Dr. Kelton will walk through the procedure, answer any final questions, and confirm your treatment plan.
- Pre-Treatment Preparation: Vital signs are recorded. The treatment area is prepared and sterilized. For joint injections, ultrasound guidance equipment is set up to ensure precise delivery.
- Cell Preparation: Your WJ-MSC preparation is confirmed for viability and potency on the day of treatment. Cells that do not meet our quality threshold are not administered.
- Ultrasound-Guided Injection: For musculoskeletal conditions, cells are delivered under real-time ultrasound guidance to ensure accurate placement within the target tissue. For systemic conditions, intravenous delivery is used. The procedure itself takes 15–30 minutes depending on the delivery method.
- Post-Injection Observation: You will be monitored for 30–60 minutes following administration. This is standard protocol to confirm there is no immediate adverse reaction.
- Discharge Instructions: You will receive written post-treatment instructions covering activity restrictions, medications to avoid, symptoms to watch for, and your follow-up schedule.
Phase 3: First 48 Hours
What Is Normal — and What Is Not
The first 48 hours are a rest-and-monitor period. Your body is beginning to respond to the cells.
Normal symptoms you may experience:
- Mild soreness or aching at the injection site — similar to post-injection soreness from any injection
- Mild swelling or warmth at the treatment area
- Fatigue — your immune system is responding
- Temporary increase in pain before improvement begins (this is a normal inflammatory response)
What to do in the first 48 hours:
- Rest. Avoid strenuous activity, heavy lifting, and high-impact movement.
- Ice (not heat) the injection site if swollen — 15 minutes on, 15 minutes off.
- Avoid NSAIDs (ibuprofen, naproxen) for at least 2 weeks — they suppress the inflammatory signaling that initiates healing.
- Stay hydrated. Avoid alcohol for 72 hours.
- Do not apply topical steroids to the treatment area.
Call us immediately if you experience:
- Fever above 101°F (38.3°C)
- Severe or rapidly worsening pain at the injection site
- Redness spreading beyond the injection site with warmth
- Signs of systemic infection: chills, rigors, confusion
- Difficulty breathing or chest tightness (seek emergency care immediately)
Phase 4: Weeks 1–4
Paracrine Signaling Initiates
This is the foundational phase. The cells you received are secreting their bioactive payload — growth factors, cytokines, and extracellular vesicles are beginning to signal your body’s repair systems. You may not feel dramatic changes yet. This is normal and expected.
- Weeks 1–2: Rest is essential. Avoid high-impact activity, heavy resistance training, and anything that loads the treated area significantly. Gentle walking is appropriate for most patients.
- Weeks 2–4: Gradual return to gentle movement. Light stretching, low-impact activity, and physical therapy (if prescribed) can begin. Listen to your body — mild discomfort during movement is acceptable; sharp or worsening pain is not.
- Typical experience: Many patients notice reduced morning stiffness and improved sleep quality in this phase. Some notice nothing yet — this does not mean the therapy is not working. Biological processes operate on their own timeline.
- Medications: Continue all prescribed medications unless specifically discussed with Dr. Kelton. Do not discontinue any medication without physician guidance.
Phase 5: Months 1–3
The Active Healing Phase
This is typically when patients begin to notice meaningful, consistent improvement. The paracrine cascade is in full effect — inflammation is being suppressed, new blood vessels are forming, and tissue repair is underway.
- Pain Reduction: Most patients experience 50–80% reduction in baseline pain scores during this phase. The reduction is often gradual and cumulative — not a single dramatic shift.
- Functional Improvement: Range of motion, strength, and endurance begin returning. Activities that were previously painful or impossible become accessible again.
- Activity Progression: Gradual return to normal activities. Physical therapy during this phase significantly amplifies outcomes — the biological environment is primed for rehabilitation.
- Autoimmune patients: Flare frequency and severity typically decrease. Fatigue often improves. Some patients begin to notice reduced medication requirements — discuss any medication changes with your prescribing physician before making adjustments.
- Neurological patients: Sensory improvements often precede motor improvements. Neuropathic pain reduction is frequently the first measurable change.
Phase 6: Months 3–6
Structural Changes & Maximum Benefit
This is the peak therapeutic window. Structural tissue changes — cartilage matrix remodeling, disc hydration restoration, nerve remyelination, vascular remodeling — are occurring at the cellular level. Many patients reach their maximum benefit during this phase.
- Pain Reduction: Further reduction from Phase 5 baseline. Many patients report 70–90% improvement from pre-treatment pain levels.
- Structural Evidence: For patients with follow-up MRI or ultrasound, measurable structural changes may be visible — cartilage thickness, disc height, tendon organization, or wound closure.
- Durability: Improvements achieved in this phase tend to be durable. Unlike cortisone injections (which wear off in weeks), the structural changes from WJ-MSC therapy are built into the tissue.
- Return to Activity: Most patients can return to full activity — including sport, exercise, and physically demanding work — during this phase, with appropriate guidance.
Phase 7: Month 6 and Beyond
Durable Outcomes & Long-Term Strategy
The goal of WJ-MSC therapy is not just short-term relief — it is durable biological change. Phase 7 is about maintaining and building on the gains achieved.
- Durable Outcomes: Most patients maintain significant improvement at 12 months. Long-term follow-up data (2–5 years) shows sustained benefit in the majority of responders.
- Maintenance Strategies: Anti-inflammatory lifestyle practices — diet, exercise, sleep, stress management — support and extend the therapeutic benefit. We provide specific guidance based on your condition.
- When to Consider Re-Treatment: Some conditions (advanced OA, progressive neurological disease, chronic autoimmune conditions) benefit from repeat treatment at 12–18 months. We evaluate re-treatment need based on your outcome tracking data, not a fixed schedule.
- Outcome Tracking: We follow up at 1, 3, 6, and 12 months with standardized outcome measures. Your data contributes to our protocol refinement and the broader evidence base for regenerative medicine.
Honest Expectations: Experience vs. Reality
